Résumé
Can you work approximately 30 hours per week at $23/hour? List your weekday availability, earliest start date, and any limits on occasional residential coverage.
How many years of residential, healthcare, administrative or operations experience do you have? Describe relevant duties and confirm whether you have a high school diploma or GED.
Describe your experience following up with applicants, scheduling interviews, organizing orientation, and tracking missing employee documents or training. How would you handle a new hire who is not cleared to start?
Which Google Sheets, Excel, email or record systems have you used? Explain how you track deadlines and missing items. Give one example of following an assignment through to a documented result without repeated reminders.
Do you have a valid driver license and are you willing to transport residents after required clearance? Describe your readiness for home checks, admission checklists, grocery/supply pickup and delivery, documentation, maintenance follow-up, and trained residential coverage. Do not include license numbers or resident information.
Select the DSP shift you prefer. Describe any scheduling limits in Relevant Experience.
Can you work the posted 12-hour residential shifts? Explain any limitations in Relevant Experience.
Describe your relevant direct care or clinical experience, training, and the population you have served. Do not include client names or private information.
